To open your account with Polylok, a completed, signed credit application will be required. Please read the following instructions carefully:
- DO NOT TRY TO FILL OUT THE CREDIT APPLICATION ON-LINE.
- Read all instructions before proceding to the next page.
- Click on the link below to download the Credit Application.
- When the Credit Application is displayed, click on the 'Print' Button on your browser.
- Return to this form.
- Completely fill out the printed form, and fax to 203-284-3495 on the top of the form.
- You will need Adobe Acrobat Reader to download the application. If you do not have the reader you can download it from the link below.
You may need to download Adobe Acrobat Reader before the account application will open.
